Clomipramine

Clomipramine review by Medicalook.com

Ever wondered why there are people who are obsessive and are now popularly known to have an obsessive compulsive disorder (OCD)? Wonder if there is a cure? Let Clomipramine help you with this.

This is a frequently prescribed drug for OCD treatment but has more side effects that the SSRI’s which prevents serotonin from being reabsorbed by the never cell and consequently helps prolong that light feeling that the brain needs. This function is acknowledged to relieve depression and anxiety.

Defined as the 3-chloro derivative of imipramine, it is seriously potent but not exactly an SSRI because the primary active metabolite desmethylclomipramine acts as a norepinephrine reuptake inhibitor.

Aside from the said indications, it is also used for panic attacks panic attacks with or without agoraphobia, narcolepsy, premature ejaculation, chronic pain and enuresis. Similar to other SSRI’s, it also takes 2- 3 weeks for the patient to experience the full benefits of the drug.

Though said to have more side effects that SSRI’s, it is considered as second-line treatment should your SSRI fail. It might even be slightly more effective as far as treating the symptoms of OCD are concerned. Curiously enough, Clomipramine and SSRI’s (Paroxetine) are considered as potent treatments for premature ejaculation.

One of the downsides of the drug is that it has a higher incident of seizures than other tricyclic anti-depressants. But even is this is the case, it is touted t be slightly better in the treatment of depression as compared to other tricyclics.

As a non-controlled substance, there is a potential for its abuse or dependence. Its withdrawal symptoms occur once you abruptly stop its use. Such symptoms include agitation, fatigue, nausea, headaches, insomnia, sometimes activation of mania and rebound of depression or anxiety. If there is a need to immediately terminate its use, benzodiazepines should be taken up to four weeks (if needed) to suppress the unpleasant symptoms.

Like most drugs with relative effects, there’s also a list of side effects associated with the drug which includes fatigue, dizziness, lightheadedness, headaches, confusion, agitation, insomnia, nightmares, increased anxiety, and seizures (0,5% to 2%, see above)

It also has anticholingeric side effects such as dry mouth, constipation, rarely ileus (paralysis of the large intestine, life-threatening), difficulties in urinating, sweating, precipitation of glaucoma (may lead to permanent eye-damage or even blindness, if untreated).

Because of the strong central and peripheral blockage of alpha receptors, antiadrenergic side effects might also be experienced such as hypotension, postural collapse (when patient is rising too fast from lying or sitting position to standing), arrhythmias (sinus tachycardia, bradycardia, AV block, rarely other forms of cardiac problems). Preexisting heart insufficiency can be worsened

Those suffering from emotional instability should receive the smallest amount of the drug. It is also advised to be cautious with its use on elderly people because they are more prone to the effects of the drug. Pregnant women should only use this drug if clearly need because it is excreted into breast milk.
